19-20 Registration - Powell Elementary

Pre-Kindergarten
Your child must be 4 years of age by September 1, 2019, to enroll for Pre-Kindergarten. This program is only available to those who qualify based on Limited English Proficiency, Homeless, ActiveMember of the Armed Forces, Educationally Disadvantaged, receiving Medicaid Assistance or Foster Child Placement through CPS. This is a half day program and could be either in the mornings from 7:50-11:00am or afternoons from 12:00 to 3:10 pm. The determination of class is based on your address. For those applying based on Limited English Proficiency, a verbal test will be administered once all registration requirements have been submitted.

Kindergarten
Your child must be 5 years of age by September 1, 2019, to enroll for Kindergarten. Per the TEA (Texas Education Agency), we are unable to offer any competency testing if your child does not meet the age requirement.

Grades 1st-4th
Please follow the directions to complete registration below and be sure to include prior school report card when completing registration.

Registration Steps

a) Child’s Original State Issued Birth Certificate
b) Child’s Original Social Security Card or Passport
c) Child’s Complete Immunization Record

4 DTap – Last shot must be on or after the 4th birthday
3 Polio (IPV) – Last shot must be on or after the 4th birthday
2 MMR – Both must be after the 1st birthday
3 Hep B – This is not the same as HIB
2 Hep A – Both must be on or after the 1st birthday
2 Varicella (Varivax) – 2 doses OR documentation of disease by a parent or Physician

d) Proof of Residency – Entire Lease if renting, OR current utility bill (water/trash, electricity or gas only), OR HUD Settlement Statement if recently purchased house and utilities not yet set up.
e) ID of adult registering the child
f) Report Card and W/D papers from Previous School (Elementary School/not private Preschools)
